Care Coordinator

SureCare (Reading & East Berkshire) - Reading

Duties include:

To carry out Service User needs assessments, care/support plans and risk assessmentsBid social service care packages and actively look for private care packages, participate in business developmentTo schedule client needs and staff availability and produce weekly rota sheets detailing all visits. Dealing with timesheetsInterviewing potential Care/Support Workers, ensuring all applicant paperwork is fully completed as per requirement.Writing up references for all Care Worker applicants and verifying references on returnTo ensure that effective induction, supervision and assessment of staff is carried out and that training needs are identified and met. Carry out spot checks as requiredLiaise with Social Services, client's and their families.Ensuring that holiday/sickness and emergency calls are assigned.To promote the service to other agencies and document all relevant information.Ensuring client's and staff paper and electronic files are kept up to datePreparation of reports as requiredDealing with emergency situationsCompleting care calls on an adhoc basisTo undertake the duty on-call on rota basis.Other responsibilities as required.

The successful candidate must be a driver with access to their own vehicle.

Qualifications & Training:

NVQ/QCF Level 2 in Health and social care or higher

Experience / Knowledge a) At least 1 years’ experience in senior care role or as care coordinator b) Knowledge of Care Standards c) Experience of CQC inspection processes d) Knowledge of issues involved in adult abuse prevention
