Exercise Instructor

Part Time  for Serco in Suffolk

SCH is a partnership between Serco, South Essex Partnership University NHS Foundation Trust and Community Dental Services CIC. Suffolk Community Healthcare are currently recruiting for an Exercise Instructor to support cardiac rehabilitation exercise programmes as part of the Cardiac Rehabilitation Team across Suffolk. To work within the community, liaising with the local hospitals, tertiary centres, local healthcare teams, GPs and practice nurses managing cardiac rehabilitation referrals at the Single Point of Access.

Duties and Responsibilities:

- Give information, advice and support about individuals conditions to patients, relatives and carers

- Maintain and foster links with leisure centres, acute hospitals, tertiary centres, district nurses and cardiology departments

- Ensure patients are referred in a timely manner to the hospitals cardiac rehabilitation programmes

- Be a resource able to provide expertise relating to cardiac rehabilitation

- Ensure maintenance of accurate and legible written and electronic records in accordance with Suffolk Community Healthcare standards and in compliance with Confidentiality of Information policy

- To attend updates and attend meetings relating to cardiac rehabilitation

Skills:

-A full driving licence is essential

-Have worked within a similar role

-Promote, monitor and maintain best practice to ensure the health and safety of patients

- Work within the lone working policy

- Act at all times in a confidential manner

- Comply with Infection Control policies

- Contribute to and participate in the planning and development of the of the cardiac rehabilitation service

- Set up and deliver cardiac rehabilitation exercise programmes

- Engage actively in practice development, evaluation, audit and research

- Prioritise own workload according to team objectives

- Be aware of budgetary constraints and work within them

- Respect the individuality, values, cultural and religious diversity of clients

- Co-ordinate the gym based exercise sessions in collaboration with the cardiac rehabilitation team

- Identify and assess patient`s risk factors in relation to coronary heart disease using recognised tools

- Formulate an individual plan of cardiac rehabilitation in collaboration with each client taking into account individual`s ability and understanding in order to make lifestyle changes
