B5 Cardiac Rehabilitation Nurse (part time)

Our vision is for a flagship service to support the shift of higher acuity care from acute hospitals into the community setting. This includes clinical interventions that will prevent hospital admissions and that will facilitate patients being cared for earlier in their recovery following hospital admission in the community setting.
An opportunity has arisen for a band 5 Nurse to join the Community Heart Failure and Cardiac Rehab Services team on a part time basis.
Ideally, applicants should have cardiac experience but training will be given to the right candidate if necessary.
The successful candidate will predominantly be based in the West covering Newmarket , Sudbury, and Mildenhall area , although there may be occasions when there is a requirement to cover the rest of the county.
The main purpose of the role is to provide cardiac rehabilitation support and advice for patients and their carers following cardiac events such as Myocardial Infarction, Angioplasty or Coronary Artery Bypass surgery.
Duties will include:To identify and assess patients risk factors in relation to coronary heart disease using recognised toolsTo formulate an individual plan of cardiac rehabilitation in collaboration with each client taking into account individuals ability and understanding in order to make lifestyle changesTo assess psychological function using recognised assessment scores e.g HAD score, Dartmouth scoreTo assist senior staff with provision of education classes to support and advise patients with their rehabilitation following coronary events
